Croup develops in more than 80 000 Canadian children each year, making it the second most common cause of respiratory distress in the first decade of life. 1, 2 It affects boys more than girls (1.4:1) and young children between 6 months and 3 years of age more commonly than younger infants, older children and adolescents. 2 Croup is frequently preceded by 24-72 hours of nonspecific cough (See ‘Initial treatment’ below and “Croup: Pharmacologic and supportive interventions”.) The approach to the management of croup will be discussed below. The clinical features and evaluation of croup, and the evidence supporting the use of the pharmacologic and supportive interventions included below are discussed separately.
Croup is common in young children, especially between the ages of 6 months and 3 years. Some children have two or more bouts of croup in their childhood. The viral infection can pass from person to person, especially if there is close contact. Croup often occurs in outbreaks or epidemics in the winter. Croup is uncommon after the age of 6 years.
